Expert panel on cost analysis of atrial fibrillation

Abstract: Objective: To estimate total cost of atrial fibrillation (AF) management concerning acute coronary syndrome, heart failure, stroke and drug related adverse events with respect to clinical practice and available guidelines.Methods: This cost analysis study was based on identification of total costs related to management of acute coronary syndrome, heart failure, stroke and the drug related adverse events in patients with AF based on standardized questionnaire forms filled by experts according to their daily clinical practice and also to ACCF/AHA/ESC guidelines. Total cost included cost items related to treatment, healthcare resources utilization, and diagnostic test and consultations.Results: The yearly cost of acute coronary syndrome per patient was 5.478.43 TL according to expert’s view reflecting real clinical practice whereas it was 11.319.44 TL when calculation was based on recommendations in the guidelines. The average total cost of heart failure was 4.523.74 TL according to expert’s view whereas it was 2.925.86 TL based on guidelines. The average total cost of stroke was 5.719.25 TL according to expert’s view but 7.931.18 TL based on guidelines. Among drug related adverse events, only those related to cardiac adverse events were estimated to be higher according to expert view as compared to guideline recommendations (288.65 vs. 150.99 TL). Conclusions: Reflecting the treatment algorithms in the management of AF and related adverse events, our findings seem to emphasize the extra burden on health economics posed by patients suffering from the uncontrolled disease.

Abstract: The aim of this study is to explore the causality relationship between Foreign DirectInvestment and economic growth in Turkey, which has liberalized foreign capital inflowsespecially after 1980s. Granger causality analysis was used in order to test the hypotheses aboutthe presence of causality between Foreign Direct Investment and Economic Growth. The study,which used the quarterly data covering the period between 1992:1 and 2006:3, showed causalityrelationship from Foreign Direct Investment to Economic Growth in Turkey. In other words,there is a one-way relationship between Foreign Direct Investment and Economic Growth andthe direction of this relationship is from Foreign Direct Investment to Economic Growth
Critical thinking trends of nurses: An example of a training and research hospital

Abstract: Introduction: The modern nursing understanding requires ability to easily adapt to the improvements and innovations in nursing, to search and question the knowledge, to be able to address problems and also critical thinking. Critical thinkingis a process which includes research, intuition, logicandexperiencewithuniversalvaluesandpotentiallyaffectpatientcare. Aim:Theaim of thisstudywastoidentifythe Critical ThinkingDisposition of Nurses in a research and training hospital and the factors that affect their disposition and to guide regulatory studies and improvements. Methods:This descriptive and analytical study included 106 nurses who were actively working in June 2014 and volunteered to participate. Data were colllected by ‘Descriptive Characteristics Questionnaire’ and ‘California Critical Thinking Disposition Inventory (CCTDI)’and analysed by Independent-Samples T Test and Oneway (ANOVA). Results:The mean age of nurses was 29.39 years. 42.5% of nurses had under graduate degree, 24.5% worked in cardio vascular surgery intensive care unit, 23.9% attended in-service training at least once outside this hospital and 28.3% attended in-service training at least once in this hospital. The mean critical thinking scores was found to be 218,188. Conclusion:The critical thinking disposition scores of emergency room nurses was higher than the others. Literature follow-up and scientific studies were found to affect the critical thinking dispositions positively
Rapidly progressive tetraplegia and cognitive deterioration during rehabilitation: A case of neurodegenerative disease

Abstract: Human prion diseases are fatal, progressive neurodegenerative disorders caused by neurolytic pathogen proteins, called prions. The most common human prion disease is sporadic Creutzfeldt-Jakob disease, with an approximate annual prevalence of 0.5-1 per million. The symptoms and signs include rapidly progressive dementia, ataxia, myoclonic seizures, akinetic mutism and other neurological and neurobehavioral disorders. The clinical spectrum of Creutzfeldt-Jakob disease is highly variable; therefore it can be difficult to diagnose premortem. This article describes a 78-year-old woman who initially presented with difficulty walking and balance disorder. As a result of the evaluation, the patient was transferred to rehabilitation clinic, with a diagnosis of cervical spinal stenosis. During hospitalization, she showed progressive decline in gait and balance and deteriorated rapidly. The patient was considered to be probable sporadic Creutzfeldt-Jakob disease after further investigations
Forgotten Subjects of Violence Against Women: Children in Women’s Shelters

Abstract: Violence against women and children, as one of the forms of violence, is a very important problem in Turkey as it is seen in many part of the world. Children, who have to witness the violence against women as the subject of violence and who are sometimes exposed to various forms of violence, and their mothers take shelter in women’s shelters as an important particinstrument to intervene in violence against women. This study aims to reveal the types of services and support programs (psychological, economical, health and education services, etc) provided for children staying in women’s shelters subjected to local administrations in Ankara and the qualifications of these provided services. In this scope, the in-depth interviews were made with 43 women staying in four women’s shelters subjected to different local administrations in Ankara by using semi-structured questionnaire forms. Also, the assessments of staff of these women’s shelters about the services given for children in women’s shelters were included in this study. The most important outcomes of this research are that the qualification of many services and support such as psychological, economical, health and education services for children in the women’s shelter in which the research was conducted is not sufficient, the women are prioritized and the children are the forgotten subjects in the women’s shelter. These insufficient support and services prevent children staying in this women’s shelter from showing their own potential. In this study, it is considered that this barrier and risk condition is the ‘structural violence’ called by Galtung and the violence against children, who are the forgotten subjects in women’s shelter, may be considered in this frame
Baseline Demographic, Clinical and Laboratory Parameters Related with 24 Hour Urinary Sodium Excretion in Newly Diagnosed Patients with Type 2 Diabetes

Abstract: OBJECTIVE: Hypertension is very frequently observed in patients with Type 2 diabetes. Increased salt intake has been shown to be related with elevated blood pressure. However, factors related with 24 hour urinary sodium (Na) excretion as a measure of Na intake is not specifically addressed in patients with newly diagnosed Type 2 diabetic patients.MATERIAL and METHODS: All patients underwent history taking, physical examination, blood pressure measurement, electocardiography, biochemical analysis, spot urine analysis, 24-hour urine collection to measure 24-hour urinary albumin, protein, sodium excretion and creatinine clearance.RESULTS: In total, 114 patients were enrolled. Spearman correlation analysis revealed that 24-hour urinary Na excretion was correlated with body mass index (BMI) ( rho: 0.265, p: 0.004). blood urea nitrogen (rho: -0.210, p: 0.025) creatinine clearance (rho: 0.313, p: 0.001), albumin (rho: 0.320, p: 0.001), hemoglobin (rho: 0.242, p: 0.013) and triglyceride (rho: 0.261, p: 0.008). Linear regression of independent factors revealed that BMI (B: 0.013, CI: 0.004-0.022, P: 0.004), presence of smoking (B: 0.132, CI: 0.02-0.243, P: 0.021), creatinine clearance (B: 0.002, CI: 0.001-0.004, P: 0.012), and triglyceride levels (B: 0.017, CI: 0.009-0.056, P: 0.003) were related with logarithmically converted 24-hour Na excretion.CONCLUSION: We demonstrated that BMI, creatinine clearance and serum triglyceride levels were independently associated with 24-hour urinary Na excretion in newly diagnosed Type 2 diabetic patients.
The Relationship Between Parathyroid Hormone, Insulin Resistance and Blood Pressure in Newly Diagnosed Hypertensive Patients with Various Levels of Kidney Function

Abstract: OBJECTIVE: There are scarce data regarding the relationship between insulin resistance, parathyroid hormone (PTH) and blood pressure (BP) in the literature. This study is aimed to investigate the interrelationships between BPs, insulin resistance and PTH levels in never treated newly diagnosed essential hypertensive patients.MATERIAL and METHODS: All patients underwent history taking, physical examination, BP measurement, 12 lead electocardiographic evaluations, routine urine analysis, biochemical analysis and 24-hour urine collection to measure protein excretion and creatinine clearance. Insulin resistance was calculated by homeostasis model assessment (HOMA) index.RESULTS: In total 92 patients were included. Spearman correlation analysis revealed that systolic BP was correlated with diastolic BP (rho: 0.337, p: 0.001), with age (rho: 0.214, p: 0.041) with body mass index (rho: 0.325, p: 0.004), with blood urea nitrogen (rho: 0.262, p: 0.012), with HOMA-INDEX (rho: 0.273, p: 0.009) and with insulin levels (rho: 0.262, p: 0.012). There was no correlation between PTH and systolic BP. Spearman correlation analysis revealed that diastolic BP was correlated with fasting blood glucose (rho: 0.220, p: 0.035), and PTH levels (rho: 0.235, p: 0.024).CONCLUSION: In conclusion, we suggest that although PTH and HOMA-INDEX are related with BPs, discrepancy exists regarding systolic and diastolic BPs.
Chemical compositions of thin-disk, high-metallicity red horizontal-branch field stars

Abstract: We present a detailed abundance analysis and atmospheric parameters of 76 stars from a survey to identify field Galactic red horizontal-branch (RHB) stars. High-resolution echelle spectra (R\simeq60,000, S/N>=100) were obtained with 2.7 m Smith Telescope at McDonald Observatory. The target stars were selected only by color and parallax information. Overall metallicities and relative abundances of proton-capture elements (C I, N I, O I, Li I), alpha-elements (Ca I and Si I), and neutron-capture elements (Eu II and La II) were determined by either equivalent width or synthetic spectrum analyses. We used CN features at 7995-8040 {\AA} region in order to determine 12^C/13^C ratios of our targets. Investigation of the evolutionary stages, using spectroscopic T_eff and log g values along with derived 12^C/13^C ratios, revealed the presence of 18 probable RHB stars in our sample. We also derived kinematics of the stars with available distance information. Taking into account both the kinematics and probable evolutionary stages, we conclude that our sample contains five thick disk and 13 thin disk RHB stars. Up until now, RHB stars have been considered as members of the thick disk, and were expected to have large space velocities and sub-solar metallicities. However, our sample is dominated by low velocity solar-metallicity RHB stars; their existence cannot be easily explained with standard stellar evolution.
The Effect of R&D and Innovation Policies Applied in Turkey between the Years of 2005-2015 on SMSes: Sample of Konya Manufacturing Sector on Agricultural Tools and Machinery

Abstract: In order to be able to stand up in today’s rapidly changing competitive environment, firms should constantly change and renew their products, services and production methods. Manufacturing of agricultural equipment and machinery is one of the sectors mostly impressed by technological developments. Smart agricultural machinery having technological equipment facilitates farmers’ work, preserves the environment and increases efficiency. In this study aiming at presenting the impact of R&D and innovation policies applied in Turkey between 2005 and 2015 on SMEs in the sample of the sector of manufacturing of agricultural equipment and machinery in Konya, recognitions of the notions of science, technology, R&D and innovation and various innovation indicators are given and technology and innovation policies applied on SMEs are mentioned by handling Turkey’s policies on science, technology, R&D and innovation in terms of historical process. In the section of application of the study, results of the survey made with 101 firms being active in the sector of manufacturing of agricultural equipment and machinery in Konya. Accordingly, 89 out of the firms participated in the survey don’t find R&D and innovation incentives applied in Turkey sufficient. It is seen that 57 firms have performed innovative activities in the last five years. Research findings suggest that the policies to be carried out to increase SMEs’ levels of technological capability are of huge importance
How Much Do We Know About Cardiopulmonary Resuscitation?

Abstract: study, we aimed to investigate how much the physicians knew about resuscitation and the current guidelines were applied during the resuscitation. Materials and Methods: A total of 134 physicians working in the training and research hospitals, the university hospitals and 112 emergency medical services. A survey including questions about the participants’ resuscitation knowledge, age, specialty, the university graduated from and the number of cardiopulmonary resuscitations (CPR)carried out in a month was performed. The level of knowledge of the physicians working in the training and research hospitals, the university hospitals and 112 emergency medical services were compared. Results: We determined that, while 24.6% (n=33) of the participants applied the ILCOR protocols correctly, 38.8% (n=52) of them applied the protocols incorrectly or incompletely and 24.6% (n=33) of them did not know the protocols. The evaluation was performed according to the physician’s work place and the best results were obtained from the university hospital physicians, but the success rate was achieved in only around 50%. This rate was even lower in the training and research hospital and 112 emergency medical services groups. Conclusion: By reviewing the cardiopulmonary resuscitation training system in Turkey, implementation of resuscitation protocols with more current information should be provided. We believe that it is necessary to establish the in-service training programs, including the emergency medical system, as a part of training for using current information in daily practice.
